Medication is a double edged sword in health care. When used properly it can be crucial to the treatment plan, however, when it is misused can cause numerous problems for the patient. One in twenty five hospitalized patients suffer serious consequences from medication error. While the medication error rates in hospital ranges from 4.4 to 59.1% worldwide, reporting of medication errors is less than 5% (Sravya, Raghuram, Lakshmi & Tejaswi, 2013). It is true a majority of these medication errors cause no harm to patients but when it does it can be a matter of life and death, perhaps, drastically changing somebody’s life.
