1.0 Introduction According Little Oxford medication is define as a medicinal drug or treatment using drugs while error define as mistake, condition of being morally wrong or degree of inaccuracy in calculation or measurement. Medication error is preventable events that occur due to an error in the process of prescribing, dispensing and administrating. According The National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP, 2015) defines “medication error as any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in control of the healthcare professional, client or consumer”. Some might feel not to be honest when there is no harmful effect or no consequences disclosure and feel will make a different if be honest. Instance of disclosure will no empower the patient and causes diminish trust and distress patient. Some will prefer be honest to maintain better confidence and relationship trust. In negative consequences, disclosures help to preserve trust relationship. Example when family of patient comes to suspect something wrong with patient’s health, better to tell truth as healthcare. It less burdened healthcare by keeping secret and maintain a good relationship between patient and healthcare provider. A fourth ethical issue is the impact on the quality of patients care. Error happen may be suggested that there are not sufficient staff, environment factor and deficiencies of system. This environment may lead to unhealthy quality care as public expects to receive a good quality care and treatment. Example when an error issue identified in hospital, it will impact badly on the reputation of hospital. In fact patient will avoid seeking treatment.
