Sarah presented as sad when communicating about her family history, interaction with others, and marriage dissolution. She displayed appropriate feelings to her past and current experiences relating to substance abuse, isolation and feelings of failure. Sarah states that she has been experiencing moods of depression, guilt and anxiety for over 6 months. She understands how her current issues contribute to symptoms she is experiencing. She does not display or report neuro-vegetative symptoms. Sarah does not report having homicidal ideations or improper judgements. Individuals with scores similar to Sarah’s on the BDI are frequently described as moderately depressed. She self reports being pessimistic and having a lot of failures. Also, Sarah indicates that she lost interest in areas she previously was interested and other people. Moreover, she reports increased crying and feelings of irritability. Sarah records that she has experienced a decline in sleep, concentration, appetite, and interest in sex. Secondly, Sarah will identify 10 negative thoughts and replace them with positive thoughts to increase anxiety and moods of depression. Thirdly, Sarah will learn at least 5 coping skills, including stress management and relaxation such as deep breathing and relaxation for daily implementation to decrease her symptoms relating to moods of depression. Fourthly, Sarah will implement positive self-talk into her daily regime for the next six months to decrease feelings of worthlessness and increase her self-concept.
