On August 23, 2005, Hurricane Katrina formed as a tropical storm off the coast of the Bahamas. Seven days later, many citizens living on the Gulf Coast would experience what is now considered to be the nation’s largest avoidable catastrophe in the 21st century. The question that remains is “Where did we fail”?
