The Patient’s HbA1C was monitored daily was at 6.5% indicating moderate risk at the time of discharge. Sally was provided individual therapy for her depression and life style coaching to manage her depression and diabetes. Sally’s treatment plan was presented at ground rounds for quality of care issues and to ensure proper care. The Behavioral Health Professional (BHP) discussed Sally’s assessment scores weekly with the treating Physician, discharge plan includes referral to specialty mental health and coordination with the Primary Care Provider. The BHP obtained release of information from Sally and faxed her CHRT, PHQ-9 and HbA1c to the Behavioral Health Consultant and Primary Care Provider at her medical home.
