Axis Ⅰ Social Anxiety Disorder and Major Depressive Disorder are the orders relate to the patient. The reasoning behind Social Anxiety is that he is afraid of being in any kind of big crowds like eating lunch at a table of many people in the cafeteria and presentations he has to give in some required classes. Troy starts to get very worried weeks before the presentation is even coming up, this distract him from studying for his other classes. The symptoms include “Depressed mood most of the day, Markedly diminished interest in all or almost all daily activities, Insomnia nearly everyday, Feeling worthlessness or excessive or inappropriate guilt, Diminished ability to think or concentrate nearly everyday (213 Butcher,Hooley,Mineka 8-29-15)”. AxisⅡ Troy does not have any diagnosis. AxisⅢ Troy’s doctor has been treating him for frequent migraines and has sleeping problems. Axis Ⅳ Troy is in his graduate degree program which is highly demanding to get your master. His past history of being always a shy person, low self image of himself, and afraid of being embarrass.
