This provider began seeing Mr. Conklin for treatment and therapy on 7.22.15. Mr. David Berg from DSHS, DDA, contacted this provider to provide treatment and therapy for Mr. Conklin. Mr. Berg informed this provider that Mr. Conklin’s previous provider had retired and Mr. Conklin was in need of continued treatment and therapy. It should be noted that that shortly after this provider began seeing Mr. Conklin, caseworker Mr. Berg transferred this case to Yvonne Alexander. Mr. Conklin was found guilty of attempted rape in the second degree x2 in 1981 for which he was incarcerated in the Washington State Department of Corrections Prison system for about 10 years. Conklin has had several incidents this reporting period that have raised concerns about his safety, ranging from hygiene issues, sexual innuendos and gestures to staff and peers, and self-harm. Each time this has been brought to the attention of this provider and discussed with Mr. Conklin, his response has been incongruent, smiling and laughing about the issues, and dismissing the event(s) altogether. It is unclear at this time if Mr. Conklin is not yet comfortable with the therapeutic relationship, or if this is Mr. Conklin’s normal response to serious events/incidents. To address Mr. Conklin’s impulsivity and lack of insight into his behaviors, he has been given the following exercise to complete after each remarkable incident that the staff at Ambitions discovers. Mr. Conklin is required to complete a Behavior Chain Analysis (BCA) after each incident. He will need help from a staff member at Ambitions because of his reading and cognitive limitations. If the incident involves staff, an uninvolved staff should be the one that assists him in completing the BCA. He will be asked to bring all of the BCA’s he has completed for the week and discuss them during our individual treatment sessions. This exercise should give Mr. Conklin more insight into his impulsive behaviors and find ways to exit the chain before they become dangerous and/or
