It is true that everyone makes mistakes. However, I ensure that I learn from those errors and avoiding going to the same anchoring trap again. I can accomplish this by empowering myself with education, an open mind and systematic approach to clinical decision making including researching and evidence-based approach.
I agree with you on the debiasing strategies you listed in your response, particularly with regards to exploring an alternative hypothesis. I think for me this is crucial because when I see the same type of diagnosis my tendency is to latched to that particular conclusion instead of actively seeking for an alternative possibility. Having said that, I always try to look at things differently
